Person-Centred Support
When developing a support package, we always start with the person and develop an appropriate service around them.  For some people with more complex needs, they may require a different legal framework of support to that of registered care.   Many people who come to our registered care services find that this is a short to medium term placement for them as they move towards increasing levels of independence.  In these services, we support people with complex needs who may have a learning disability, mental health support need or a combination of support needs.  The support team work to a supported living philosophy and the emphasis is on enabling people to live full lives with choice and control.   Referrals often come from formal or secure settings, we work with the individual and the commissioning body to develop packages that will meet people’s desired outcomes and increase their independent living skills.  24 hour care is provided so there is someone always available and care packages can be increased or reduced to suit the needs of the people we support.  Each person has a bespoke care package, tailored to meet their needs, which is reviewed on a regular basis.   We can offer multi-occupancy or single-person care homes which can be adapted accordingly.  Get in touch with the team to find out more.
